We service patients that have had a major hospitalization or
condition change that require follow-up care from a nurse or need
rehabilitation but want this care to be given at home. Studies have shown that
patients recover faster and have better outcomes at home then in a facility.
Our ultimate goal is for the client to remain safely in their home and to
prevent future hospitalization.
Extended Health Care, Inc. was founded over 20 years ago with the mission to provide quality nursing care at home. We provide traditional visiting nursing and rehab services for clients that have been recently discharged from a hospital or nursing home and require follow-up care. Our nurses provide follow-up education on your newly prescribed medications and teach clients about their newly diagnosed chronic condition. Other cases may require continuation of IV medication or wound care at home. Our skilled therapists provide physical, occupational and speech therapy in the most ideal setting – your home.
Private duty nursing is a special service covered by Medi-Cal for clients who want to live in their own home but need a licensed nurse or certified home health aide in the home. Depending on the client’s condition, the client can qualify for a skilled nurse to stay at home with the patient for up to 16 hours a day. The care is covered under Medi-Cal with no cost to the patient.

Certified Home Health Aide: A highly skilled Home Health Aide provides assistance with grooming and bathing in the home. Aides can also provide light housekeeping and meal preparation for our patients.
Medical Social Worker: Licensed clinicians that understand that help patients and their families cope with the changes associated with a major hospitalization or condition change. Our social workers will provide vital connections with transportation, Meals-on-Wheels, community out-reach programs and groups that can help a patient safely remain at home at a low to no cost to the patient.
Private Duty Nursing – Certified Home Health Aide: The private duty aide provides personal care, bathing, meal preparation and light housekeeping in the home but stays in the home up to 7 hours/day 3-7 days/week. This is a special program funded by Medi-Cal for qualified applicants.
Private Duty Nursing – Skilled Nursing: A licensed nurse provides one to one skilled nursing care in the patient’s home for 8 – 16 hours a day. We specialize in ventilator, trach and/or tube feed dependent patients. But this is only a few of the skilled care we can provide at home. This is a special program funded by Medi-Cal for qualified applicants.
Private Duty Nursing – Case Management: A registered nurse comes to the home on a monthly basis to assess the patient’s medical condition and medical regime. We help the patient navigate and coordinate their medical care with all their different physicians and health care providers.

- reconcile the patient’s home medication with medications prescribed at discharge to prevent accidental overdose or re-hospitalization from medication errors
- teach the patient ways to prevent future hospitalization by learning effective strategies to better manage chronic or newly diagnosed conditions
- ultimately to teach the patient to manage their chronic conditions safely at home
- continue administering IV medication or complex wound care at home
Rehab Services: Therapy provided by a licensed physical, occupational or speech therapist at home. They continue the care provided at the hospital or facility in the home setting.
- Physical Therapy: provides a treatment plan to improve or
maintain current muscle strength and balance. In addition, the physical
therapist will do a home safety evaluation, teach you how to effectively use
your assistive device in your home and teach you skills to safely live at home
injury-free.
- Occupational Therapy: teach patients how to complete daily
grooming or household chores effectively so patients are more independent at
home.
- Speech Therapist: teach patients with speech and swallow impairments,
exercises to improve muscle strength in the mouth and throat.
